ABSTRACT

Objective To observe the abnormality of lipid metabolism and renal function change in elderly patients with hypertension,and to evaluate the relationship between atherogenic index of plasma(AIP) and glomerular filtration rate(GFR).Methods From July 2015 to June 2017,in Quzhou People's Hospital,182 elderly patients with hypertension were selected as observation group,and 192 elderly healthy people with normal blood pressure were selected as control group.The total cholesterol(TC),triglyeride(TG),high density lipoprotein cholesterol (HDL-C) were detected,and the AIP =1og [TG/HDL-C] was calculated.The GFR was estimated by Cockcroft-Gault equation.The relationship between the estimated GFR (eGFR) and the blood lipid parameters was analyzed.Results The eGFR of the observation group and control group were (84.94 ± 19.49) mL · min-1 · (1.73m2)-1,(154.91 ± 20.44) mL· min-1 · (1.73m2)-1,respectively,the difference between the two groups was statistically significant (t =11.27,P < 0.01).The AIP of the observation group and control group were (0.68 ± 0.03),(-0.22 ± 0.02),respectively,the difference between the two groups was statistically significant (t =3.43,P < 0.01).There was a negative correlation between AIP and eGFR in the elderly patients with hypertension (r =-0.845,P < 0.01).Conclusion The fact shows that abnormality of lipid metabolism in the elderly patients with hypertension is present.AIP can be used as a plasma marker of atherosclerosis and could be used as a useful and important parameter for clinical observation in the elderly hypertensive patients with renal impairment.

ABSTRACT

Objective@#To investigate the correlation between hyperuricemia and non-frozen cold wounds (chilblains) in agricultural workers of a township for prevention measures.@*Methods@#From October 2016 to September 2017, a cross sectional study was applied to 3107 agricultural workers of a township in kaihua county, zhejiang province, of which 371 people with hyperuricemia were observed as the objective group and of which 2 736 people without hyperuricemia as the control group. The data of the occurrence of non-frozen cold wounds (chilblains) in winter among the objective group and the control group were gathered. The descriptive statistics was carried out with the prevalence ratio (PR) of chilblains and 95% confidence intervals of those agricultural workers with hyperuricemia.@*Results@#The prevalence rate was 11.94% and 371 cases with hyperuricemia among 3107 agricultural workers of a township. The total number of cases of non-frozen cold wounds (chilblains) was 195 cases and the prevalence rate was 6.28% with Chilblains among 3 107 agricultural workers of a township. The total of 55 cases with chilblains (14.82%) among the objective group, and 140 cases with chilblains (5.12%) were found among the control group (χ2=52.342, P<0.05) . The objective group had a high prevalence of chilblains. The prevalence ratio (PR) of chilblains was 2.897 (95%CI 2.163-3.881) .@*Conclusion@#Hyperuricemia may be a risk factor for non-frozen cold wounds (chilblains) . Those agricultural workers with hyperuricemia should pay more attention to keep warm and prevent non-frozen cold wounds (chilblains) in winter.

ABSTRACT

Objective To explore the pathological changes of liver fibrosis of patients with schistosomiasis liver fibrosis.Methods Between January 2003 and January 2009,microstructure retrospective observation of liver biopsy pathological paraffin specimens by optical microscope was carried out in 9 cases of patients with schistosomiasis liver fibrosis in Quzhou people's hospital internal medicine.Then those paraffin blocks were changed to resin embedding blocks and superfine slices were made.Ultrastructure observation by electronic microscope was carried out.Results Visible fibrous scar formation and fiber calcified nodule eggs in microscopic structure of liver tissue in 9 cases of patients with schistosomiasis liver fibrosis were observed.Ultrastructural observation of the 9 cases of patients with schistosomiasis liver fibrosis showed that most of the liver cell profile was clear,liver cell nucleus gap expanded,the number of rough endoplasmic reticulum decreased,swollen mitochondria crest occurred,collagen deposited around hepatic sinus and surroundings and collagen fiber bundle around small veins extended to hepatic lobule and surroundings,and calcified nodule eggs were not easy to find.Conclusions Under an electron microscope,diseased tissue cells can be observed at subcellular level or organelle level,which provides more insight into the pathology of schistosomiasis liver fibrosis.Calcified egg nodules are easily found by an optical microscope other than an electron microscopy.The two can complement each other.

ABSTRACT

Objective To assess the quality of life (QoL) of rural elderly persons using the health-related dimensions of the EuroQol-5 (EQ-5D) scale to analyze the scale's reliability and validity.Methods A total of 2575 elderly residents of Tongchun rural community in Kaihua county,Zhejiang Province,were surveyed using the instrument EQ-5D,and the visual analogue scale of EQ.They included 1027 with and 1548 without chronic disease.Results The mean total EQ-5D score was 6.49 ± 1.94 for the chronic disease group and 5.15 ± 0.44 for the healthy group.The corresponding average visual analogue scale (VAS) scores were 78.15 ± 17.26 and 91.47 ± 4.37,showing significant differences.About 75% of the elderly respondents reported no difficulties in their living.There was strong correlation between the EQ-VAS scores and the scores on all the EQ-5D scale dimensions.Conclusion The health-related quality of life reported by the rural elderly using this scale seems quite good.The EQ-5D scale,with the EQ-VAS scale as a part of it,is one useful and effective tool for evaluating the health-related quality of life of the elderly in rural areas.

ABSTRACT

The fall risk was evaluated with Modified Falls Efficacy Scale (Chinese version) in 426 elderly hypertensive patients (hypertensive group) and 436 elderly people without hypertension (control group) from rural communities in Zhejiang Quzhou areas.The fall risk scores were compared between hypertensive group and control group ; and also between patients with history of falls and those without falls.The results showed that the average score was 8.62 ± 2.15 in hypertensive group and 9.35 ± 1.35 in control group (P < 0.01).The average score was 6.94 ± 1.85 in hypertensive patients with history of falls (n =112) and 9.22 ± 1.42 in without history of falls (n =314) (P <0.01).Our study suggests that there is a higher fall risk in elderly hypertensive patients than in elderly people without hypertension; and fall risk assessment with Modified Falls Efficacy Scale (Chinese version) can be used for prediction of fall for the elderly people in rural community.

ABSTRACT

To discuss research topic selection of basic general medicea and the implementation of the project,so as to improve health service level and quality in rural communities,the case of Kaihua County in the western zhejiang Quzhou city was explored.The case showed that research topic selection and project implementation of general medicine is feasible and fruitful in rural communities.And it is beneficial to advance growth of talents general medicine and improve the quality of rural community health service.
